Privacy Practices
Privacy and confidentiality are central to substance use disorder treatment. This page explains our general approach and how this website handles information.
This is a general summary, not a legal notice. Aeterna Health's formal Notice of Privacy Practices and applicable 42 CFR Part 2 and HIPAA disclosures should be supplied and published here before launch.
Confidentiality of treatment information
Substance use disorder treatment records are protected by federal and state confidentiality requirements. Information about a person's participation in treatment is generally not disclosed without written authorization, except in limited circumstances permitted by law.
Residents receive detailed information about their privacy rights, authorizations and how records may be used and disclosed as part of the admission process.
Information collected through this website
This website collects only the information you choose to submit through the contact form: your name, a way to reach you, your role and a brief message.
The contact form intentionally does not ask for diagnoses, medications, substance use history, dates of birth, insurance identifiers or other sensitive details. Please do not include them.
Website messages are not a secure channel for protected health information. Sensitive information should be shared by phone or another secure method arranged with our team.
